Job Title: Patient Navigator

Job Summary:

This position provides clear communication offering The Holding Group services to new patient referrals and community partners, while tracking and documenting across multiple electronic platforms. This position collaborates with palliative care social workers for effective patient outreach, care coordination, and handovers across the continuum of care. Refers patients and provides educational resources to patients and families per protocols.

Key Responsibilities:
  • Work with internal and external resources to provide support and coordinate follow-up care.
  • Assist patients and their families in connecting with insurers, payer programs, and other resources to meet social and clinical needs.
  • Provide resource counseling to patients and their families.
  • Maintain strict adherence to HIPAA regulations to protect patient confidentiality and privacy.
  • Manage, organize, and update relevant data using database applications.
  • Communicate effectively and efficiently across all care levels and disciplines.

Work Location:

Remote – this role is eligible to work remotely, but the employee must be in the United States.

Qualifications:

Minimum Qualifications:
  • A bachelor's degree in health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.

A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year for year basis.

Applicants must meet minimum qualifications at the time of hire.

Preferred Qualifications:
  • One (1) or more years of relevant professional healthcare experience.
  • One (1) or more years of palliative care, hospice care, or case management experience.

Knowledge, Skills, and Abilities:
  • Ability to communicate effectively, both in writing and orally.
  • Ability to establish and maintain effective working relationships with employees at all levels throughout the institution.
  • Excellent interpersonal skills.
  • Outstanding customer service skills.
  • Excellent critical thinking and problem-solving skills.
  • Strong mediation skills.
  • Ability to work with individuals from diverse backgrounds.
  • Ability to work with minimal supervision.
  • Proficiency with and comfortable using a personal computer and computer programs such as Microsoft Word, Excel, PowerPoint and Zoom.
  • Proficiency with and comfortable using a smartphone.
