Job Summary:
This position will provide direct clinical services to patients, and those involved in their care, who are facing serious illness, through consultative visits and telephonic outreach that facilitates advance care planning discussions and offers information/education regarding their illness trajectory and treatment options. Interventions will be delivered via telehealth to patients in a variety of care settings, including sub-acute rehab facilities and the home.

- A bachelor's degree in health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
- Possess a valid licensed Clinical Social Worker (LCSW) certification in the State of Colorado.
Intermediate Level:
- A bachelor's degree in health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
- One (1) year of relevant professional healthcare experience.
- Possess a valid licensed Clinical Social Worker (LCSW) certification in the State of Colorado.
Senior Level:

- Two (2) years of relevant professional healthcare experience.
